Norfolk driver confirms this. The 51 year old driver was killed when he
climbed onto the hopper to clear material that was jamming the compaction
unit in the truck. According to the report, factors that contributed to his death
include:
1. He failed to turn off the truck before entering the hopper.
2. He ignored warnings on the hopper door reminding him to remove the keys
from the ignition.
3. A safety switch that turns off the power takeoff unit when the hopper door
is open had been bypassed.
4. A safety switch that turns off the truck’s hydraulic system when the hopper
door is open had been bypassed.
5. The truck was a back-up vehicle and a different type of truck than his usual
truck.
6. He was not trained on the safety and other aspects of the truck.
Please perform thorough pre-trips on all vehicles and make sure safety switches
are not bypassed.
